Student Ministry Forms

MEDICAL RELEASE FORM 2008 - this form must be on file for any trip that your student goes on with Istrouma's Student Ministry. Forms remain on file for one calendar year and are valid for all activities for that year. We only keep notarized forms, so if the form is not notarized, you will have to fill out another form for the next event. 

VOLUNTEER MEDICAL RELEASE FORM 2008 - this form must be on file for all adults who chaperone any of our trips. It must also be notarized and will be valid for one year.

VOLUNTEER CRIMINAL BACKGROUND CHECK FORM - all adults who volunteer in our ministry must file for a criminal background check. Simply fill out this form and bring it get it back to us. We'll send it off to the police department. Thank you for cooperating as this ensures the security of our students.

SCHOLARSHIP APPLICATION - we don't ever want money to keep your student from participating. At the same time, we want to be good stewards of the resources God has blessed us with. We ask that you help us by helping your student be creative in possibly raising money by mowing lawns in the neighborhood, babysitting, etc. This might even open up opportunities for your student to share Christ with those who let them earn money for their trip.
