- IBC Medical Release Form (NOTE: A notarized Medical Release Form must be on file for children who attend camps, retreats, trips off campus, etc. The form must be updated each year, and is good for the entire calendar year.)
- Rok Haus Waiver (for 5th Grade Retreat)

- Judson Retreat Center Camper Form (must be completed for Extreme Walk Camp)
